Who do I contact if my company is not paying the health insurance or union fees? I pay into these?

They take out money out of my check for health insurance and union dues. But every few months I get a letter from the insurance company saying I have no insurance. The same is for my union dues, I get a letter stating they have not received dues from my company. Who do I file an anonymous report to for this deliberate fraud?
